Summary of Bovie Medical Corporation Benefits

HEALTHCARE PLAN: Coverage is available for full time employees and eligible dependents at a minimal cost paid by the employee. The healthcare plan is a Preferred Provider Organization which has In-network and Out-of-network benefits. In-network benefits will allow you to pay a co-pay and you are required to use an in-network provider. If you visit an Out-of-network provider you will be subject to a deductible and co-insurance. The co-pay and co-insurance is determined by the Plan Option chosen. If you wish to utilize out of network providers it will be necessary to sign up for the PPO Plan, while using only in network providers, your choice would be our HMO Open Access Plan. The Open Access HMO Plan offers you the ability to choose any providers in the directory without obtaining referrals.

DENTAL PLAN: Coverage is available for all full-time employees and eligible dependents. The dental plan offers two options: You have the choices between a DMO and a PPO Plan. With the DMO, you must stay in network; with the PPO you have the choice of staying in-network or going out-of-network at a slightly higher cost.

LIFE INSURANCE: Bovie Medical Corporation pays the full costs of Basic Life Insurance and Accidental Death and Dismemberment for all full time employees. The amount of coverage covered for an employee is $15,000. You have the option of buying additional life insurance for yourself and your dependents. You can buy $10,000 increments up to a maximum of $500,000 for yourself. You may purchase up to $250,000 for your spouse in $5,000 increments. You can purchase up to $10,000 for each of your children, in $2,000 increments (Spouse and children can equal no more than 50% of your amount).

LONG TERM DISABILITY: Bovie Medical Corporation pays the full costs of Long Term Disability for all full time employees. The benefit is 66 2/3% of your prior monthly earnings not the exceed $5.000. Normally benefits will continue the length of disability or age 65.

SHORT TERM DISABILITY: Non Exempt Employees who are absent from work due to a non-occupational illness or injury may be eligible for up to 11 weeks of disability pay. Prudential will pay you 60% of your basic weekly earnings (up to $500.00 a week) after a 14 day elimination period. This plan premium is paid by the employee.

RETIREMENT SAVINGS PLAN (401K): Bovie Medical Corporation offers a 401(k) Plan through payroll deductions. Employees may contribute between 1% and 25% of eligible pay on a pre-tax basis up to the annual IRS dollar amount.

VISION CARE PLAN: A vision care plan is offered to all full time employees and their eligible dependents through our Healthcare Plan. Coverage provides for routine eye exams and eyeglasses or contact lenses at a reduced rate.

CANCER PROTECTION PLAN: Employees are eligible to sign up for our Cancer Protection Plan. The cost is fully paid for by the employee. The plan provides benefits if Cancer is diagnosed for hospital confinement, radiation, chemotherapy, nursing services, surgery, anesthesia, prostheses, drugs, etc. under the terms of the plan. Family coverage is available.

PERSONAL ACCIDENT PROTECTION PLAN: The Personal Accidental Indemnity Plan is a supplement, it is not intended to cover the medical expenses of your injuries, our healthcare plan will do that. This policy pays you direct! The cost is paid for by the employee.

HOSPITAL INDEMINITY PLAN: This plan is a supplement. The policy pays you directly for any in- hospital stay. Full information is available under the terms of the Plan. The cost is paid for by the employee.

PREPAID LEGAL PLAN: This plan is available to all employees and their eligible dependents. Cost for this Plan is the responsibility of the employee. Prepaid Legal Services offers identity theft protection and restoration as well as family legal plans.

LIFESTYLES FAMILY FITNESS PROGRAM: All full time employees are eligible for enrollment in the Lifestyles Family Fitness Centers. Cost to the employee is $2.31 per week.

EMPLOYEE ASSISTANCE PLAN: Bovie Medical Corporation pays for an employee assistance plan for employees and their dependents. This Plan is voluntary, confidential counseling service. A wide variety of help is available under this plan, such as, financial, legal, child/eldercare, marriage counseling, etc. These are just some services available under this Plan. You and each of your eligible dependents are permitted up to three visits per year.

EDUCATIONAL ASSISTANCE: Bovie Medical Corporation offers tuition reimbursement for continuing education at an accredited college or university. The benefit is available to full time employees after three months of service.

PAID TIME OFF: Bovie Medical offers eight (8) paid holidays per year. The vacation schedule is as follows: 1-5 years – 2 weeks, 5+ years – 3 weeks.

CAFETERIA PLANS: The Cafeteria Plan (Section 125) allows you to make your eligible premiums on a pre-tax basis, thereby reducing an employee’s tax liability while increasing your take home pay.
